By electronically signing and Submitting this form, you agree to the following:

You authorize Kapa Technologies, LLC to debit regularly scheduled charges to your card used for the initial payment or any debit or credit card provided in the near future to replace card on file.

Payments are for the services provided by Kapa Technologies in placing a GPS Monitoring Device on defendant mentioned who has been required by the courts and/or has voluntarily entered an alternative to Incarceration Electronic Monitoring Program. First monthly payment plus a one time service fee will be debited the day of install and activation of the monitoring device.  

Initial Payment (First Month $280 + One time service fee of $150): $430.00

Monthly Charge (all future payments beginning month two): $280.00

** Monthly Recurring payments will start One day before the activation of GPS Device. For example: activation day was October 5th. Recurring payments will start on Month two which in this case would be November 4th.

Fees Non-Refundable: All initial fees (monthly or service fee) paid on this agreement are Non-Refundable with the exception that the monitoring device was never activated and installed on the defendant. Recurring monthly payments are non-refundable and non-transferable.      

An additional install fee (minimum of $75) may be assessed if a replacement monitoring device is required as a result of a cut strap, submersion, or intentional damage to any of the devices components. An uninstall (removal) fee may also be accessed to remove device from the defendant once it is no longer required (minimum of $75).  You also understand that you will be held responsible for damages, other than normal wear, to the equipment. If the equipment is not in good working condition you will be charged for the repair/replacement of the equipment as follows:

Monitoring Device: $1,500.00

Secure Cuff: up to $150.00

Charging cord: $30.00

Charger pigtail: $20.00

You understand that this authorization will remain in effect until the GPS Monitoring device is removed from the defendant.  You certify that you are an authorized user of this Debit or Credit Card and will not dispute these scheduled transactions; so long as the transactions correspond to the terms indicated in this authorization agreement.
